Our Training Program

In MINT Memory Clinics, each health care professional has received standardized accredited training to deliver
high-quality dementia care.

The MINT Memory Clinic training program is an award-winning, standardized dementia care training program for interdisciplinary healthcare professionals in primary care. This program aims to improve the detection and management of dementia through the development of self-sustaining primary care-based memory clinics.

Over 100 primary care sites and over 2,000 healthcare professionals in five provinces across Canada have completed the MINT Memory Clinic training program to date.

The MINT Memory Clinic Model of Care

From the first referral to follow-up, MINT Memory Clinic teams work within a shared care approach with the patient's family physician. Patients with memory concerns are referred by their family physician to a MINT Memory Clinic for comprehensive assessment and care planning. The MINT Memory Clinic provides a detailed and proactive care plan to the family physician, and refers to specialists when necessary.
